Amyotrophic Lateral Sclerosis

Progressive neurological disease causing loss of muscle strength, mobility, speech, swallowing, and breathing function.

HIV

Advanced HIV disease with progressive decline, recurrent infections, weight loss, and increasing care needs.

Pulmonary Disease

Advanced lung disease with worsening shortness of breath, declining function, and increasing need for supportive care.

Stroke & Coma

Severe neurological impairment following stroke or coma with major loss of function, communication, and independence.
