What Is Continuous Home Care?
Continuous Home Care (CHC) is a short-term level of hospice care provided during a symptom crisis when a patient requires extended nursing care to remain at home.
This level of care is designed to:
Stabilize acute symptoms
Prevent unnecessary hospitalization
Maintain comfort in the home setting
CHC is temporary and only provided when medically necessary.




When Is Continuous Home Care Needed?
CHC is initiated when a patient experiences a crisis period, such as:
Severe, uncontrolled pain
Acute respiratory distress
Uncontrolled nausea or vomiting
Anxiety or agitation requiring intensive monitoring
Rapid decline requiring frequent clinical intervention
The goal is to bring symptoms under control so the patient can safely return to Routine Home Care.

Note: CHC is not provided in inpatient hospice units or hospitals.


How Long Does Continuous Home Care Last?
CHC is short-term and reassessed daily:
Provided only during active crisis periods
Discontinued once symptoms are stabilized
Transitioned back to Routine Home Care when appropriate
Who Qualifies for Continuous Home Care?
To qualify, patients must:
Be experiencing an acute symptom crisis
Require continuous nursing intervention to remain at home
Have care that is reasonable and necessary under hospice guidelines
Care Team Involved
Continuous Home Care is coordinated by the hospice interdisciplinary team:
Physician / Medical Director
Registered Nurse (primary provider during CHC)
Hospice Aides (as supportive care)
Social Worker / Spiritual Counselor (as needed)
All services are directed under an individualized Plan of Care (POC) and closely monitored.
